    Eligibility Criteria:

    You must meet the following eligibility criteria to apply for Equicom Gold Credit Card:

    • You must be between 18 and 70 years of age
    • You must be a resident of the Philippines
    • You must earn a minimum gross income of Php 500,000 per annum. You must be able to verify your annual income earned by submitting necessary documents.

    Documentation Requirement:

    You must submit the following documents to be eligible to apply for Equicom Gold Credit Card:

    • The completed application form with your signature.
    • Income Proof Documents – According to the BSP Circular No.622, you must submit your latest Income Tax Return with a Bank/BIR stamp or BIR form No.2316 AND the following documents:
      • Salaried employees must submit any 1 of the following documents which must not be older than 3 months:
        • Employment Certificate – must contain your hiring date, compensation and position
        • Appointment Letter
        • Employment Contract
        • Salary Increase notices, if applicable
        • Most recent Payslip – must show your 1 month’s income
        • Most recent Credit Card Statement
      • Self-employed applicants must submit ALL of the following documents:
        • Your most recent Tax Return document for a full year.
        • Financial statements (must be audited).
        • A copy of your registration with the DTI, SEC or any appropriate government agency.
        • If your business is not listed in the top 5,000 corporations, you must additionally submit your bank deposit certificate or your latest credit card statement.
      • Foreigners must submit a copy of any 1 of the following documents:
        • Your Philippines Visa
        • Alien Certificate of Registration stating which type of Visa you hold
        • ICR or Immigration Certificate of Residence
        • Note: The documents mentioned above must hold a minimum validity of 1 year from your application date.
    • ID Proof documents – you must submit a copy of any 2 of the following ID proof (front and back copy) documents:
