    Insurance Offered

    Credit Life Insurance: In case of your temporary or permanent disability or your unfortunate death, your outstanding balance will be paid off by credit life insurance in the following ways:

    • If you have suffered temporary disability – Credit Life Insurance will take care of your monthly installments by paying P500 or 5% of your outstanding balance, depending on whichever of the two is higher.
    • If you are permanently disabled or in an event of your unfortunate death - Credit Life Insurance will cover your outstanding balance up to P400,000.

    You have the option to renew this insurance policy up to the age of 60 years. The approval of your policy will depend on factors including your credit card being in good standing and your ability to keep up the payments on your insurance premiums.

    Supplementary Card

    • Get unlimited number of supplementary cards for your friends and family.
    • The annual fee for each supplementary card is P750.
    • The supplementary cardholder must be 14 years of age and above.

    Eligibility Criteria

    To apply for this credit card, you must meet the following eligibility criteria:

    • Age criteria:
      • Minimum age – 21 years.
      • Maximum age – 60 years.
    • Income criterion – Minimum annual income of P600,000.
    • You must have a business or residential landline phone.
    • You must be employed, practicing a profession or be a business owner with your business operational for a minimum period of 1 year.

    Documents Required

    You must provide the following documents for applying for this credit card:

    • Photocopy of any 2 ID proof documents that contain your signature AND 3 specimen signatures.
    • Income proof document for employed applicants/fixed salary earners:
      • BIR Form 2316 (W-2).
    • Income proof documents for self-employed applicants:
      • Income Tax Return (ITR) BIR Form 1701 or 1702 containing a bank/BIR stamp.
      • Business Name Registration with the Department of Trade and Industry Certificate.
    • If you are residing outside Metro Manila, you must also submit your latest proof of billing address.

    If you have lost or misplaced your UnionBank Assumption Alumnae Association Visa Card, or it has been stolen, you must immediately contact the bank and notify them of the loss/theft. Depending on your location, you can contact the bank through the following ways:

    Furthermore, you will be charged P400 as a Lost Card Replacement Fee for the bank to replace your lost/stolen credit card.
