    Insurance Offered

    Credit Life Insurance:

    In an event of your temporary disability, permanent disability or death, credit life insurance will cover the outstanding balance on your credit card.

    • In case of your temporary disability, this insurance cover will make monthly instalment payments of P500 or 5% of your outstanding balance, depending on whichever of the 2 is higher.
    • In case of your permanent disability or death, credit life insurance cover will pay your outstanding balance up to P400,000.
    • You can renew the credit life insurance policy until the age of 60 years. Your renewal will be subject to your credit card being active and in good standing, as well as your ability to keep up with the premiums.

    Supplementary Cards

    • You can apply for unlimited number of supplementary cards.
    • The supplementary cardholder must be at least 14 years of age.
    • The annual membership fee for each supplementary card is P750.

    Documents Required

    You must submit the following documents to the bank to complete your application process:

    • Photocopy of any 2 ID proof documents that contain your signature AND 3 specimen signatures.
    • Income proof document for employed applicants/fixed salary earners:
      • BIR Form 2316 (W-2).
    • Income proof documents for self-employed applicants:
      • Income Tax Return (ITR) BIR Form 1701 or 1702 containing a bank/BIR stamp.
      • Business Name Registration with the Department of Trade and Industry Certificate.
    • If you are residing outside Metro Manila, you must also submit your latest proof of billing address.

    How to apply for Lourdes School Alumni Association Visa Card

    You can apply for this credit card through one of the following 3 ways:

    Submit an online application to the bank:

    • Visit UnionBank’s website and navigate to the Lourdes School Alumni Association Visa Card webpage.
    • Scroll down to the bottom of this page where you will find the option, “Apply Now”.
    • Click on “Fill out an online application form” available under this option.
    • Complete the online application form and submit the applicable documents requested through email, mail or fax.

    Lost/Stolen Cards Liability

    In an event that your Lourdes School Alumni Association Visa Card is lost/misplaced or stolen, you must immediately call the bank and inform them of the loss/theft.

    Your credit card has a feature called “Lost Card Protection”. This feature will relieve you of any financial liability in case your card is used fraudulently. But, this feature will only be applicable after you have notified the bank regarding the loss/theft of your credit card.

    The bank will charge you a Lost Card Replacement Fee of P400, to replace your lost/stolen credit card.
